Five Areas Experts Assess Patient Condition <\/p>\n
Ukrainians who require constant outside care due to age, disability, or health conditions can receive such services from the state.<\/p>\n

For such cases, Ukraine has a mechanism for determining the need for constant outside care. This includes assistance with getting out of bed, dressing, eating, or orienting oneself in space.<\/p>\n Who and how determines the need for care<\/p>\n In Ukraine, there are two main paths to confirm such a need, depending on the individual’s situation:<\/p>\n Expert teams (ECOPOF)<\/strong> determine the need for care when assessing daily functioning. This applies to those who are applying for the first time<\/strong> to establish disability or undergoing a re-examination<\/strong>. In this case, the treating physician must specify the goal in the referral: to determine the need for constant outside care.<\/p>\n Medical advisory commissions (VKK).<\/strong> If the disability status is already established<\/strong>, but the person only needs documentary confirmation of the need for care for social procedures, this can be done through the VKK. The commission issues a conclusion in the form of No. 080-4\/o<\/strong>.<\/p>\n What ECOPOF determines<\/p>\n The expert team consists of practicing doctors from various specialties<\/strong>. They analyze the actual functional capabilities of the person<\/strong>, not just the presence of a diagnosis or disability group. The key task is to establish whether the person can independently, safely, and without life-threatening risks meet basic needs.<\/p>\n The following factors are taken into account:<\/p>\n If a person experiences severe pain or health risks<\/strong> when performing certain actions, it is considered that such actions cannot be performed independently. If the expert team establishes the need for care, this must be indicated in the decision following the assessment.<\/p>\n How criteria are assessed in practice<\/p>\n The team checks whether the person can perform actions consistently and safely across five areas<\/strong>:<\/p>\n Conditions for establishing the need for care<\/p>\n The commission’s decision depends on the degree of limitations. The need for constant outside care is established if:<\/p>\n The Ministry of Health notes that a pronounced (2nd degree) limitation in only one criterion<\/strong> is usually not sufficient grounds for establishing the need for care. Such a need can be established for people with disabilities of group IA, IB, as well as group II<\/strong> provided the corresponding degrees of limitation.<\/p>\n Following the assessment, the individual receives an extract from the decision and recommendations (the medical part of the individual rehabilitation program). The documents are sent via email or registered mail.<\/p>\n What VKK determines<\/p>\n In situations where disability is already established<\/strong>, and the person only needs to document the need for care for social procedures without re-evaluating functioning, this fact is recorded by the medical advisory commission (VKK)<\/strong>.<\/p>\n The commission assesses the actual dependence of the person on the help of others, taking into account:<\/p>\n Main forms of conclusions<\/p>\n Based on the VKK’s review, one of the following documents is issued: form No. 080-4\/o or a conclusion in accordance with the Ministry of Health Order No. 667.<\/strong><\/p>\n Form No. 080-4\/o<\/strong> – a conclusion regarding the presence of functional impairments that necessitate constant outside care. It is intended for registering motor activity impairments (paralysis) and cognitive disorders in various diseases (schizophrenia, Alzheimer’s disease, Parkinson’s disease, strokes, consequences of injuries, etc.).<\/p>\n According to the updated rules<\/strong>, there is no need to specify the particular agency to which the conclusion is submitted; electronic signatures are permitted.<\/p>\n For indefinite disability, the conclusion is indefinite<\/strong>. In other cases, it is valid for the duration of the disability, but not less than 12 months<\/strong>. For individuals without disability status – for 12 months<\/strong>. Previously issued conclusions remain valid until the end of their term.<\/p>\n The conclusion according to the Ministry of Health Order No. 667<\/strong> is for individuals with disabilities of group I or II due to mental disorders.<\/p>\n Terms:<\/strong> for group I – for the duration of the disability. For group II – not less than 6 months<\/strong> (or 12 months<\/strong> if care is provided by a non-working pensioner).<\/p>\n Next steps<\/p>\n Based on the decision of the expert team or the VKK’s conclusion, the process of arranging social services and preparing documents for social protection agencies begins. Relevant information is recorded in the electronic system.<\/p>\n The Ministry of Health emphasizes that medical institutions (ECOPOF and VKK) only confirm the need for care. They do not determine the specific individual<\/strong> who should provide it. This issue is resolved during subsequent procedures for arranging social services beyond the authority of medical institutions.<\/p>\n Previously, RBC-Ukraine reported on the crisis in the care system in Ukraine. More than 3 million people need assistance, but the state covers only about 21% of this need. The remaining burden falls on families, and the situation is exacerbated by the war, staff shortages, low wages, and the absence of a unified accounting system, which threatens to deepen the problem further.<\/p>\n We also reported on the expansion of state support for veterans in Ukraine.
